1. Membership is available to anyone between the ages of 7-17 years of age.

2. You pledge that I will not be an accomplice to violence and will not condone violence in any way. I will not watch someone be bullied, picked on, put down or abused.

I pledged that I will be the voice for that person and will speak to a teacher, life coach, or our community member or an adult in SWAT. I pledged to always show kindness, I will not bully nor be disrespetcful. I will stand for what I believe and will not allow peer pressure to control me, I will not engage in any activities that condone violence nor abuse.

I will set the example for those around me, I will be respectful, honest and trustworthy. I will be a productive citizen of my community. I will be an active voice for PEACE in my community, church and home. 
We call upon responsible citizens and officers of government in the local, state and federal levels to seek out and support programs and measures designed to combat youth violence and provide a safe and secure environment for all youth within our community.

3. I will participate in fundraising activities to keep this program free to me. 

4. I will participate in Community Service Activites and will spread the information to others my age.

 

1. As a parent I agree that I will be supportive to my child as he or she participates in the program. 

2. I agree that I will not make excuses for my child not attending their scheduled meet time. 

3. I agree that I will submit a copy of my child report card every marking period to info@survivorswithatestimony.org

4. I will not condone my child participating in any violence.

5.I will be supportive of my child being a voice for others. 

6. I will stand against bullying.

7. I will participate in fundraising activities performed by SWAT to keep the program at a low rate for my child/ren and the community.

8. I do understand that it is a yearly application fee of $75 for one child and $40 per additional children living in the same household.  The yearly cycle starts over every September. 

9. I will be that adult that will call upon responsible citizens and officers of government in the local, state and federal levels to seek out and support NU CREATIONS YOUTH programs and measures designed to combat youth violence and provide a safe and secure environment for all youth within our community.

 

 

1. As a Mentor, Tutor or Volunteer of Survivors With A Testimony I agree that I will be supportive to all the  children that participates in the program. 

2. I agree that I will not make excuses and will make scheduled mentor and tutoring appointments and if I am going to be late the parent will be notified in writing and with a phone call. 

4. I will report any know activity of my mentee participating in any violence.

5.I will be supportive of my child assigned to me.

6. I will stand against bullying.

7. I will participate in fundraising activities performed by SWAT to keep the program at a low rate for the child/ren we serve in the community.

8. I do understand that it I will not discuss any payments with the parents or accept any cash for my services SWAT will give me a monthly stipend. 

9. I will call upon responsible citizens and officers of government in the local, state and federal levels to seek out and support NU CREATIONS YOUTH programs and measures designed to combat youth violence and provide a safe and secure environment for all youth within the community.
