ADMISSIONS GUIDELINES
Enrollment Process and Entrance Requirements
Dr. Bose Community Center requires that applicants complete the following process:
1) Interview with an Admissions Representative
2) Applicants must be at least 18 years of age.
3) Cannot be convicted of a criminal offense listed in Texas Health & Safety Code 250.006
4) Cannot be listed as unemployable on Employee Misconduct Registry (EMR)
Dr. Bose Community Center does not discriminate on the basis of race, color, national origin, sex, religion, age, veteran status, or disability in admissions or access to its training programs or activities. However Dr. Bose Community Center reserves the right to refuse service to anyone as long as they are not for the aforementioned reasons.
Admission Procedures
After satisfying the admissions requirements, members will complete a Tuition Free Enrollment Packet. Dr. Bose Community Center is a Community Based Tuition Free Training Facility where members will not be required at any time to pay a tuition for training programs.
HEALTH SERVICES
Dr. Bose Community Center cannot be expected to control all of the risks associated with its programs and in case of an accident, there may be the need for emergency system response.
Dr. Bose Community Center does not provide health care for members. We will utilize the 911 emergency system in an emergency situation.
In the event that the 911 emergency system is utilized, the member will assume financial responsibility of treatment unless the incident or injury occurred due to gross negligence from Dr. Bose Community Center.
MEMBER RIGHTS AND RESPONSIBILITIES
Dr. Bose Community Center is committed to training integrity and standards of excellence of the highest quality in all training programs and an environment that fosters the educational process and well being of all members.
Dr. Bose Community Center recognizes that success is a shared responsibility between our community members and our facility. Members attending Dr. Bose Community Center are expected to accept and adhere to the following responsibilities.
1) Member attendance and participation is paramount to success. Regular and punctual attendance in class and laboratories is expected of all members.
2) Members are expected to exercise personal responsibility and self- discipline as they engage in the different segments of our programs.
3) Inappropriate behavior may result in disciplinary action/termination against the Member. (Example: Profanity, Vulgarity, Horseplay, Disrespect)
4) Members attending Dr. Bose Community Center are responsible for adhering to standards of academic integrity. Academic dishonesty may result in disciplinary action against the Member. (Example: cheating, copying, falsifying course work)
In support of the rights of its members, Dr. Bose Community Center is committed to:
Ensuring that members are treated with fundamental fairness and personal dignity that includes an impartial process for members to appeal disciplinary sanctions imposed or to file a complaint or grievance when they believe they were unjustly or improperly treated by the facility, facility employees, or other members.
Training Program Hours
Members will have one of following options:
9:00 A.M. to 12:00 P.M.
12:00 P.M. to 3:00 P.M.
3:00 P.M. to 6:00 P.M.
6:00 P.M. to 8:00 P.M.

Medical Programs Direct Number

All Medical Programs Enrollees will use this number to communicate with the faculty. No other number shall be recognized as this is the only line that the Medical Programs Staff have access too. In the even that the member calls out of office hours or there is no answer at the time of the call, the member shall leave a text message with their name and inquiry. The Medical Programs Staff will then return the members call and address the inquiry.
ATTENDANCE, RE-ENROLLMENT & TERMINATION
Members must attend every class and clinical day. The maximum amount of days allowed for absence cannot be greater than 10% (3 days) per training program. Any absence over 10% (3 days) must be made up within that week. Arrangements must be made prior to attending any make up day.
If a member arrives to a training program 15 minutes after the scheduled start time, it will be considered an absence.
Any member with more than 20% (6 days) of absenteeism without making up the corresponding hours will be terminated from the training program.
It is the Members responsibility to keep up with their attendance/absenteeism and to make arrangements for making up the required hours. Dr. Bose Community Center will not he held responsible if a Member does not meet the required attendance due to member negligence.
Any member interested in repeating a training program after termination as a result of absenteeism or any other non-disciplinary reasons, must restart new training session.
Member Conduct / Causes for Termination
Any member under the influence of alcohol, drugs, distribution, and possession of illegal substances, possession of knifes, firearms or any weapons will be terminated without possibility of readmission.
Any member involved in sexual harassment, acts of violence, or verbally abusive to other members or staff will be terminated without readmission.
Any member using profanity, vulgarity, involved in horseplay, disrespect, allegations, confronting other students, disrupting the orderly running of the program or creating an environment that is not conducive to learning may be terminated from the program.
Member failing to demonstrate academic integrity or academic dishonesty may also be terminated from the program.
MEMBER GRIEVANCES
Any grievances shall be directed to the Program Instructor unless the grievance is with such Instructor, then it shall be directed to the Facility Director in writing. The grievance will then be processed and Facility Director will meet with the member within 3 working days of date of submittal for possible resolution of matter.
UNIFORM AND ATTIRE
All Medical Program Members will be requiered to wear a Navy Blue set of Medical Scrubs approved only by Dr. Bose Community Center.

White Tennis Shoes (tennis shoes made out of cloth, canvas, mesh are not allowed)
Optional: White short sleeve/long sleeve undershirt
Joggers or any deviation from the approved Uniform attire will not be allowed and the member will not be allowed to participate in any class, lecture or clinical activity for that day.
CERTIFICATION EXAM ELIGIBILITY
In oder to be eligible to take a certification exam, the member must meet the following criteria:
1). Overall academic average above 70%
2). Minimum of 90% training program attendance
3). Completed all clinical and externship practices
4). BLS Provider CPR Certification
If a member does not meet aforementioned criteria 1 or 2, that member may not register for a certification exam and will have to complete a new training program.
If a member does not meet aforementioned criteria 3, that member must complete the required clinical and externship practices withing 30 days of the training program end date, or will be required to complete a new training program.
EKG and Phlebotomy clinical practices for members who need additional time will be held every Tuesday and Wednesday from 2:00p.m. - 4:00p.m.
Members must call the Admissions Department and be added to the schedule before they can come in and do clinical practices.
CERTIFICATION EXAM FEES AND SCHEDULING
Once a member has finished a training program and is eligible to register for the Certification Exam, the member must pay the corresponding examination fees before Dr. Bose Community Center can pre-register the member for the Certification Exam.
CERTIFICATION EXAMS WILL BE CONDUCTED ON THE SECOND AND FOURTH THURSDAY OF EVERY MONTH.
The member must pay the corresponding examination fees 3 days (by monday) prior to the training program completion date in order to test on the scheduled testing date. Once the member has payed the examination fees, it is the responsibility of the member to meet in person with the Admissions Department and pre-register for the exam.
Members who pay the examination fees but do not pre-register for the exam with the Admissions Department will not be able to sit in for the Certification Exams and will have to reschedule for a later date by filling out a State Exam Delay Request form.
Likewise, members who do not pay the corresponding exam fees will need to fill out a State Exam Delay Request form in order to test at a later date.
Members who do not take the Certification Exam on their originally scheduled date will have 60 days to take the Certification Exam or will have to repeat the training program.
CERTIFICATION EXAM RETESTING
If a member fails a Certification Exam, the member must fill out a State Exam Rescheduling form. After failing a Certification Exam the member must allow the mandatory 30 day grace period before rescheduling for another certification exam attempt.
Once the 30 day grace period has passed, the member will have 60 days to take the Certification Exam or will have to repeat the training program.
