    Please complete the cover sheet which includes identifying information. The cover sheet will not be included as a part of the actual selection process by the review committee.

    Evaluation

    Please complete the evaluation according to the directions.

    Do not include any personal identifying information regarding the applicant as applications will be scored anonymously.

    Inclusion of any identifiable information will lead to disqualification of application.

    Review of proposals is anonymous.

      The content of your evaluation will remain confidential. When you have completed the form, please press the submit button. The fom will then be sent directly to the Krum ISD Education Foundation along with a copy to the email you provided.

      Review and consideration for funding is a “blind review” by assigned Foundation committee members.  At nowhere in the application process should the applicant's name be identified.  If the applicant's name is inadvertently placed in application, application may be subject to disqualification and ineligible for review.
