Division of Housing Homeless Contribution Tax Credit - Organization Eligibility Verification

Created by Colorado House Bill 2022 - 1083 (HB22-1083), the Colorado Homeless Contribution Income Tax Credit provides a tax credit to Colorado taxpayers that contribute to eligible homelessness related projects within Colorado. Non-profit organizations must complete and submit the following Eligibility Verification Form to the Colorado Division of Housing (DOH) for approval in order to have donations eligible for the tax credit. Each non-profit organization should complete one Eligibility Verification Form per organization. Include all Project Activities, both Operational Services and Capital Campaigns, in one form. Complete an Activity Type section for each separate program. For example, complete one for each shelter if the organization has multiple shelters. Letters of Support

Attached two letters of support. (1) Attach a letter of support from the organization's board of directors or equivalent body. (2) Attach another letter of support from regional Continuum of Care or other regional homeless response coordinating entity. All letters should verify acknowledgement of and support for the organization's participation in the Tax Credit program, including specific Project Activity types. Additional letters of support may be attached, but are not required.

Donation Tracking *

Project Activities, also known as Qualifying Activities, fall into two categories: Operational Services & Capital Campaigns. Operational Service means as service with the primary focus on assisting individuals or families experiencing homelessness or, in the case of prevention, individuals or families facing imminent risk of homelessness. Case Management should be selected only for services that are not provided within any other Operational Services types. Capital Campaign means a campaign that encourages public and private partnerships and is focused on raising funds for a specific capital project.
