Stateline Sports Group Indoor Facility Request Form

Please read before completing the form.

1. Please review the Indoor schedule before making your submission.

2. Please submit a different request for a 90-minute weekday request and a 120-minute weekend request.

3. If you are requesting multiple days, you need to select a date for each requested day.

4. This form will allow you to select up to 10 different dates. If you need additional dates, you will need to submit another submission form.

5. From November 1st through March 31st your team's indoor facility budget will be debited $65.00 an hour for requesting 1/2 of the facility and $135.00 an hour for requesting the full building.
