    This camp is an overnight camp in Jurong Christian Church. If you are not comfortable with your child staying overnight, please indicate below. 这将是一个在裕廊基督教会过夜的营会。如果您不想要孩子过夜，请在下面表明。

    I agree and allow my child to participate in this camp and all activities organised by the camp committee. The camp committee will do their best to ensure the safety of all participants. During the camp, if my child has any accidents or loses his/her items, I agree that Jurong Christian Church and the camp committee will not be held responsible. 我同意及准许我的孩子参加此营会及营会筹委会所举行的一切活动。营会筹委会和组长将会尽全力确保孩子们的安全。若我的孩子在营会期间发生任何意外或遗失物品，我同意、不对裕廊基督教会及营会筹委会追究任何责任。

    By filling up this form, I allow Timothy Youth Fellowship (Jurong Christian Church), her agents and authorised service providers to collect, use and disclose my data for the purpose of Administration.


    I am aware that photographs / audio recordings / video recordings of me may be taken and these may be used for internal and external publicity purposes in mediums such as printed newsletters, brochures, flyers, posters, banners, websites, and social media platforms.
