Please fill out the following information to register the delegates from your school. All forms, including guidance counselor, delegate registration, and payment forms, are ONLINE ONLY through the JotForm service. After this form is filled out, the registration form will be electronically sent via email to both the parent and student. Please be sure the emails you enter for the parent and student are NOT a school email (.edu/.org) as those systems tend to block external emails, especially automated emails sent from JotForm. Please be sure this email is checked regulary as important information will be sent out, especially in the weeks leading to seminar. You will receive an email with the link to the payment form to be sent to the responsible party for seminar fees: school, sponsoring organization, parent, etc.
