APPLICATION FOR FREE BURIAL OF POOR OR INDIGENT MUSLIMS

Procedure to be followed by Informant 

(Other than Hospital /Nursing Home/Welfare Organisation)

Step 1: Ensure that the death has been certified by a medical practitioner and that the E-DEATH CERTIFICATE is AVAILABLE

Step 2: Please contact us via mobile phone and ensure that the burial arrangements can be made. Kindly complete this form and attached the E-DEATH CERTIFICATE and other supporting documents latest by 2pm.* Otherwise, the burial will be carried out on the next day.

Conditions for free burial for the deceased: 

(a) No next of kin or/and

(b) No claimant or/and 

(c) The claimant/next of kin are unable to pay for the burial.

 All information is mandatory filled as it will be used for NEA application of permit to bury.

*Subject to our availability for burial on the same day. 

1. Please note that only the following formats are accepted: DOCX, DOC or PDF format.   

2. Each file should be less than 5 MB. 

  1. The deceased and/or next-of-kin have a current per capita household income less than $1,500.00 ;

2. The deceased and/or next –of-kin do not have savings of more than the current nisab*;

3. The deceased and/or next-of-kin is in state of financial difficulty being in constrained to pay the burial expenses incurred.

 

I make this solemn declaration by virtue of the provisions of the Oats and Declarations Act 2000 (Cap 211) and subject to the penalties provided by that Act for the making of false statements in statutory declarations, conscientiously believing the statements contained in the declaration to be true in every particular.

NOTIFICATION

By submitting this form, you hereby give your consent to Muslimin Trust Fund Association to collect, use and disclose your personal data and the deceased for the purposes of applying NEA Permit to Bury and submission to NEA. You may refer to our Data Protection Notice at our websites for detail.
