-Comply with the University’s Academic Honesty Policy
-Work with the instructor to determine test date and start time - You must enter a specific date and time
-Submit this form to the Student Access Services (SAS) at least 72 business hours prior to the test date
-Arrive at your scheduled test time. When scheduling, the end time for your exam should be no later than 5pm
-For consideration of exceptions please email proctoring@npe.nmt.edu
-If you are more than 15 minutes late you will not be allowed to test without the approval of the instructor
-Any changes that differ from the original test date and or time that is provided by the instructor must be approved by the instructor and notice given to SAS via email with proof
-If for any reason the information on your form does not comply with the requirements the SAS will reject your proctoring request and you will need to complete a new form
If you need to request proctoring later than the 72-hour window before the exam, you may request an emergency proctoring form from access@nmt.edu. You are allowed two emergency forms per semester, after that you will need to work with your instructor to be proctored at a different location if you miss the 72-hour deadline a third time.
