2023 Silver Access Application
Complete this application form after you have selected your health insurance plan.

If multpiple people are in one policy, then only 1 application is needed. If different policies in 1 family, then multiple applications will need to be completed.

You will need to have a copy of your Health Insurance Marketplace application and Eligibility Notice nearby to answer some of the questions.

After submitting your application, you will receive an email confirmation and copy of your application.

Application submitted before the 15th of the month, will recieve confirmation of approval before the end of the month. Applications submitted after the 15th, will recieve approval notice by the end of the following month.

During Open Enrollment (Nov - Jan 15):All applications submitted before 12/15 will recieve an approval or denial letter before January 1st.

As of 11/1, Silver Access is not yet contracted with Molina Health, please be aware that Silver Access is not able to confirm third party payment abilities for February premiums, but are welcome to apply to be put in a waitlist.
