You must submit this form with all presenter information in order for your abstract to be considered for presentation in the SOPHE 2023 Annual Conference . As a reminder, all presenters and co-presenters must register for the full conference by January 31st. If you do not register, SOPHE reserves the right to modify the program and re-assign your spot to another presenter. If you have any other questions, please email education@sophe.org
