PLEASE NOTE: Schools of Choice for the fall is open. All documentation needs to be received by 8/30/24 at 3 p.m. Fraser Public Schools does NOT accept out-of-county Schools of Choice applications.
The registration appointment will be at the Administration Building, 33466 Garfield Rd., Fraser, MI 48026 (located at the corner of Klein & Garfield):
- Online Registration Forms in PowerSchool (14 forms MUST be complete before appointment)
- Mortgage Statement or Property Tax Bill or Deed or Lease along with a Notarized Landlord Affidavit (all pages fully executed by all parties, not expired, and the legal guardian must be listed on the lease)
- Two Recent Billing Statements (gas, electric, cable) displaying guardian's full name and address- SHUT OFF NOTICES, INTENT TO SHUT OFF NOTICES, CELL PHONE BILLS, OR WATER BILLS WILL NOT BE ACCEPTED
- Child's Original Birth Certificate (we will make a copy and give you back the original)
- Parent ID (Driver's License or State ID)- It must must match residency documents. There are no exceptions.
- Hearing and Vision Test along with a Dental Screening (Young 5s & Kindergarten Only)
- Current Immunization Record or Waiver from the Health Department
- Last Report Card and Unofficial Transcript
- Disciplinary Record for the Past Two Years
If all documentation is NOT submitted at the appointment, the student will NOT be enrolled. All documentation must be submitted at the appointment.
