Certificate
1) I (Owner) represent that I am the legal owner of the above-named pet(s) and I assume all risks, dangers, and responsibility for injuries to named pet(s). Owner understands and agrees that Owner is solely responsible for any harm while Owner’s pet(s) is/are attending Daycare/Boarding/Grooming.
2) I (Owner) certify that my pet(s) is/are in good health with no communicable diseases and have provided proof of applicable vaccinations. Rabies, Parvo, Distemper, and Bordetella for daycare/dog boarding. Rabies and Distemper for
cat boarding.
3) I (Owner) certify that my pet(s) are flea/tick free. Should a dog be observed to have fleas and/or ticks, a flea/tick bath treatment may be applied and charged to
owner.
4) I (Owner) certify that my dog(s) has/have never harmed or threatened anyone or any animal and that my dog is being admitted based on this representation.
5) I (Owner) understand that any misrepresentation of my dog(s) will result in denial or cancellation of Mountain Side Pet Lodge services.
6) I (Owner) understand and agree that if any medical issues develop with my pet(s) while at Mountain Side Pet Lodge, they will be treated as deemed best, in Mountain Side Pet Lodge sole discretion, and I assume all financial and legal responsibility for all expenses
incurred.
7) I (Owner) give permission for Mountain Side Pet Lodge to use any pictures of my pet(s) photographed while in their care in print and/or online advertising, and on their social media outlets.
Waiver
1) I (Owner) understand and agree that during normal pet play, Owner’s pet(s) may sustain injuries. Pet play is monitored by Mountain Side Pet Lodge to best avoid injury, but scratches, punctures, torn ligaments, and other injuries may occur despite the best supervision. Owner further understands and agrees that neither Mountain Side Pet lodge nor anyone that occupies the property will be liable for any illness, injury, death, and/or escape of Owner’s pet(s) provided that reasonable care and precautions are followed, and Owner hereby releases Mountain Side Pet Lodge and those occupying the grounds of any liability of any kind whatsoever arising from or as a result of Owner’s pet(s) attending Mountain Side Pet Lodge.
2) I (Owner) understand that Mountain Side Pet Lodge is not responsible for any lost or destroyed personal items I choose to admit with my pet(s).
3) I (Owner) agree that I am solely financially and legally responsible for any injury incurred by my pet(s) or injury caused by my pet(s) while at Mountain Side Pet Lodge.
4) I (Owner) agree that if Owner’s dog(s) is/are not picked up by the closing time of 5:30pm then Owner hereby authorizes overnight boarding appropriate for dog(s) and to pay Mountain Side Pet Lodge for applicable boarding charge.
5) I (Owner) understand and agree that Mountain Side Pet Lodge will consider my pet(s) abandoned and thereby will forfeit my legal ownership of my pet(s) 10 days after my scheduled departure date of my pet(s) unless I have contacted Mountain Side Pet Lodge.
6) I understand that mountainside pet lodge provides off leash mountain hikes and that although all precautions are made to keep dogs safely by our side and in the pack, there is the chance they could run off as well as the possibility of getting injured or killed from encountering wildlife.
