This agreement releases Better Together Mesa from all liability relating to any damages or theft of your personal property that may occur while residing in the Group Home. By signing this agreement, you agree that Better Together Mesa is entirely free from any liability, including financial responsibility for any loss or damages occurred to your items. You also acknowledge the risks involved in using any personal property (Game Stations, TVs, video games, game consoles, bikes, cell phones, electronics, etc) while residing in group home.
By signing below, you forfeit all rights to bring a suit against Better Together Mesa for any reason regarding this manner. You agree to have receive full responsibility. You will also make every effort to obey safety precautions and the house rules as explained verbally and written. You agree to ask for clarification when needed.
You are also waiving the option of having your personal items stored safely in the Better Together Mesas storage facility, which you could have returned to you upon discharge. Please list item(s) description below:
