  • Retainer Tattoo Agreement Form

  • This retainer form is for clients that have made arrangements with an artist.

    DO NOT fill out the form until you have emailed and made arrangements with your preferred artist.

    They will instruct you when to fill it out.

    You can email your preferred artist from their portfolio on our website.

  • 1. All retainers are non-refundable and non-transferable.
    2. If you arrive at the allotted time and date of all of your scheduled appointments the retainer will be deducted from the price of your last appointment.
    3. If you do not show up for an appointment, show up for an appointment more than 20 minutes late, reschedule or cancel your appointment without 14 days notice, or decide to not get tattooed the retainer is forfeit.
    4. Most clients do not request a drawing revision, if a revision is needed please notify your artist at least 14 days before your appointment. In order to be able to serve all of our clients in a timely manner we are only able to do one revision to the drawing after the initial drawing, this does not include major idea changes, minor revisions only. Major revisions or idea changes may require an additional fee for drawing time.
    5. I grant Remington Tattoo and its affiliates, successors, licensees, and assigns the right to photograph, film, record, and use my image, likeness, voice, and/or performance, and/or media in any and all forms. This Media may be edited, modified, copied, exhibited, published, or distributed in any format, and used in perpetuity, without notice or compensation.
