- You are under 18 years of age. Parent/guardian consent is not accepted.
- You are pregnant and/or breastfeeding.
- You received Botox or fillers 6 months prior to the service date anywhere on the upper half of the face (eye level and above).
- You have glaucoma.
- You have diabetes mellitus.
- You have high blood pressure or mitral valve disorder.
- You have transmittable blood diseases (i.e. Hepatitis B/C, HIV/AIDS, Tuberculosis, Malaria, Syphilis, etc).
- You have an autoimmune disease.
- You have cancer and/or are currently under or scheduled for radiotherapy or chemotherapy treatment, or received treatment within the last 6 months.
- You take mandatory blood-thinning medications daily.
- You are a hemophiliac.
- You have skin conditions on or near the procedure including, but not limited to: eczema, psoriasis, scabs, cuts, cold sores, shingles, undiagnosed rashes, or blisters.
- You have moderate to severe acne on or near the procedure area.
- You have experienced post-inflammatory hyperpigmentation.
- You are allergic to pigmentation and makeup.
- You have healing disorders.
- You are prone to keloids.
- You are taking prescribed skin medications to cure acne (i.e. Accutane, Retin-A, strong retinoids, etc.) or have taken prescribed skin medications within the last year. MUST BE OFF OF SKIN MEDICATIONS FOR 1 YEAR IN ORDER TO GET PERMANENT MAKEUP DONE.
- You are epileptic or who has experienced faint spells or seizures.
- You have had previous eyebrow tattoos done (previous ombré powder or microblading) that has NOT been approved by Haylee.
Please notify us if you have/had allergic reactions to topical anesthetics.
Please notify us of any other medications you are currently taking.
Please notify us if you have a health problem or disease not listed above.
If you have other health complications or diseases not listed above, please consult your doctor to see if permanent makeup is a safe procedure for you. If approved by your doctor, please notify Haylee of your condition via call or text at (619) 254-7272. A legitimate doctor's note from a visit that is scheduled no more than one week before your appointment is required to proceed with your appointment. We do not want to put your health in jeopardy, as well as the artist's health.
If you have already booked an appointment and have to cancel due to the health contraindications, your deposit will be forfeited. If you have a temporary health contraindication (i.e. underage, pregnancy, taking Accutane, etc.), you will also have to forfeit your deposit and submit a new deposit for a future booking once you are cleared of all health contraindications. If you decide to show up to your appointment with any of these disqualifications, the service will be refused and you will be automatically charged 100% of the appointment price.
