    • We/I choose to participate with the parish communities in the Sacramental preparation process.  Together we will continue to prepare our child to celebrate sacraments of Reconciliation, Confirmation and Eucharist.

    • We/I understand that from time to time photos will be taken during workshops and ritual celebrations.  We give our consent to using these photos for purposes of communicating the events of our sacramental preparation within the parish community.  This could be through newsletters, parish magazine and/or parish website and facebook.
