    Use this form when you receive your order and one or more items is either damaged or incorrect. We will file a claim on your behalf if needed and will contact you via email or phone to rectify the situation.

    A photo showing the damaged merchandise, inside the original box, with all of the original cushioning (picture should show the placement of the merchandise and packaging inside the box).

    A close-up photo of the shipping label with tracking number (please zoom in enough to read the tracking number).

    Two photos displaying all six sides of the package (One photo should display the top and two sides. The second photo should display the bottom and the opposite sides).

    **Please do not discard your merchandise or the original packaging, UPS may want to pick up and inspect the item(s). We will contact you to let you know when it's ok to discard the item(s)

  • Please include a photo that shows the item defect, or a video if possible if that format better demonstrates the defect.
