To request a copy of Baptism, First Communion, Confirmation or Marriage certificate please fill out the information below. Per the Archdiocese, any member of the Christian faithful has the right to obtain a certificate for a sacrament he or she has received. Only that person or the parent of a minor child can make the request. Please expect 7-10 business days for requests to be processed and certificates issued.
