Indicator 13: Secondary Transition IEP Appeal Form

The Indiana Department of Education (IDOE) has identified local educational agencies (LEAs) who have a finding for Indicator 13: Secondary Transition (Transition IEPs).

In an effort to ensure Results Driven Accountability (RDA) Determinations accurately represent each LEA's performance and compliance, IDOE allows for an appeal process related to Transition IEPs that may have been found noncompliant in error and/or were unable to be corrected as part of the technical assistance (TA) process.

LEAs wishing to appeal their finding for Indicator 13 must include the following in the form below:

STN
Date of Report (Located underneath the STN in the report sent to LEA's by Indiana Secondary Transition Resource Center (INSTRC))
Reason(s) IEP was found to be noncompliant
Reason(s) LEA believes the IEP is compliant

If you wish to review an IEP with IDOE that was found noncompliant, but could not make a correction during TA, please list the STN, Date of Report, and Reason the IEP could not be corrected.
