1)     Fill out this form, including the required information for payment including ACH, and attach your W9.
a.     W9 is required to be 1099 status.

Process

2) When placing an order on our website, within the “Payment” section, choose the email address used to sign up for the SPIFF program in the “Spiff Email” dropdown to apply to the order.

If an order is not placed our website, please write the email address used to sign up for the SPIFF program on the purchase order. This can be put in the notes section on the purchase order.
