ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
각 강의등록 수업료는 8주 일시불 이며 학생의 개인사정으로 결석할 경우 credit을 드릴 수 없는점 양해 바랍니다. 각 수업시간은 55분이며 음악 클라스 (2명 정원)에서 1명 결석 시 다른 1명에게는 일대일 30분 개인수업으로 변경될수도 있는 모든 사항에 동의합니다.

그리고 첨부한 ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM 을 읽었고 모든 사항에 동의하며 아래 서명란에 싸인 합니다.

The class registration fee is a one-time payment that covers the entire 8 weeks course. Please understand that we cannot provide any credit if students are absent for any reason. Each class is 55 minutes, and if one person is absent from the music class, which has a capacity of 2 people, the class may change to a one-on-one 30-minute personal class for that day. Signing this agreement acknowledges our policy.

I certify that I have read this document, I fully understand its content, and I agree to the terms. I am aware that this is a release of liability and a contract and I sign it of my own free will.
