Authorized Agent for Medical Decisions

The Cabins at Aspen Grove is located inside Aspen Grove Veterinary Care, a full-service small animal hospital. If boarding guests become sick or injured, our highly trained, on-site medical team will begin treatment. Additional costs for medical bills may apply. Owners are responsible for complying with our payment policies.
If my pet becomes sick or injured, and me or my agent cannot be reached in a reasonable amount of time, I authorize the doctors at Aspen Grove Veterinary Care to make all medical decisions for my pet.*

Aspen Grove will not be held liable for lost or damaged property. Aspen Grove DOES provide bowls, kongs (as requested), bedding, cots, etc. Please DO NOT bring personal pet bowls. Toys can only be utilized under supervision and cannot be left overnight in the kennel.

Boarding Policies, Consent, and Liability Release
I understand the policies, consent and liability release have no expiration date and apply to all owners and/or agents of the pet(s). I have read and acknowledged these policies, and agree to abide by them for as long as I continue to use Aspen Grove for my Boarding and Day Care needs.*

If your pet will not eat during their stay, we will automatically take the following steps:

DOGS:
1. After 1st skipped meal: add water to dry food
2. After 2nd skipped meal: mix wet food (Hill's Science Diet Sensitive Stomach and Skin) with dry food
3. After 3rd skipped meal: switch to solely wet food (Hill's Science Diet Sensitive Stomach and Skin)
4. After 4th skipped meal: doctor exam and intervention (Exam fee and medication costs will apply) Owners are responsible for all associated fees.
CATS:
1. If a cat does not eat for 24 hours: we will offer wet food (Hill's Science Diet Sensitive Stomach and Skin)
2. If a cat does not eat for 48 hours: doctor exam intervention (Exam fee and medication costs will apply) Owners are responsible for all associated fees.

Diarrhea Policy.

If your pet is bright, alert, and responsive, and not exhibiting any other symptoms of concern we will start a GI (Biome) prescription diet for the first 24 hours. Food charges will apply.

If your pet is known to have diarrhea during their boarding stay, please follow up with your veterinarian before their stay to discuss preventative measures.

If diarrhea, or consistently loose stool, continues for 24-hours a doctor will examine your pet, we will run a fecal float & cytology to rule out parasites, and the doctor might prescribe medications. The doctor will also ensure your pet is not dehydrated.

Owners are responsible for paying for prescription diet, doctor exam, lab and medication costs.

If your pet has diarrhea during their stay with us, please indicate below which steps you would like for us to take.
Bright, Alert, and Responsive*

Please contact me after the 24 hour period, or my agent, if my pet is continuing to have diarrhea but is otherwise bright, alert, and responsive. If Aspen Grove does not hear back by end of day contacted, I authorize the recommended next steps.Please do not contact me, or my agent. I give my consent for treatment.I am a client of record (Aspen Grove Exam has been completed within the past 1 year) and authorize a fecal to be run after the 24 hour period if my pet's diarrhea continues. In addition to diarrhea, if your pet is showing other symptoms of concern, including but not limited to, vomiting or lethargy, your pet will be transferred to our hospital for care. Treatment might include doctor exam, fluids, fecal float & cytology, and medications. You, or your agent, will be contacted to alert you to the situation and present you with treatment options. Owners are responsible for paying for all medical care.*

Our state license requires that all medications and supplements be provided in the original packaging. Prescription medication must be in a prescription bottle (or original packaging) with a current prescription label from a licensed veterinarian. Medications or supplements provided in a plastic bag WILL NOT be administered or accepted into the building. Any over the counter MEDICATION or SUPPLEMENT MUST have a prescription label from a licensed veterinarian. This includes calming treats. MEDICATION LABEL PROVIDED ON MEDICATION WILL BE THE INSTRUCTIONS FOLLOWED AND MUST MATCH "WORD FOR WORD" BELOW. WE CANNOT GIVE OUTSIDE OF WHAT IS WRITTEN ON THE LABEL PROVIDED, NOR CAN WE GIVE BY WRITTEN UPDATES. ADJUSTMENTS TO INSTRUCTIONS MUST BE UPDATED ON THE PRESCRIPTION LABEL. Group Play Consent

To participate in group play, your dog must pass our personality assessment. Personality Assessments are a minimum of 2 hours and have a fee. This is completed Monday - Friday and can be done during stay. If dropping off during the weekend, assessments may not be able to be performed. (There is a brief fee for attempted assessments). If your pet has not been in group play in over 6 months, we can re-assess. Fees apply.

Directive and Consent for CPR or DNR

The Colorado Department of Agriculture (PACFA) requires us to have the following information on file for all boarded animals.
Many critical cases are treated here at Aspen Grove Veterinary Care. It often becomes pertinent for us to make serious medical decisions regarding your pet’s treatment. Most notably is the question, do you wish for us to resuscitate your pet in the event of cardiac or respiratory arrest? This is a serious procedure with many potential complications and added financial responsibility of up to $1,000 or more.

Animals that have survived cardiopulmonary arrest and have been successfully resuscitated (CPR) are extremely critical and unstable. Management of the post-arrest patient requires vigilant monitoring and the technical expertise of dedicated critical care personnel. This level of care can be costly and the outcome is uncertain.
Should my pet require cardiopulmonary resuscitation, I request that the doctor(s) at Aspen Grove Veterinary Care, pursue such medical care as indicated.*

YES, please perform CPR on my pet in the event of cardiac or respiratory arrest. I accept that if the hospital staff is unable to reach me within a reasonable amount of time after the initiation of CPR procedures, and after exercising reasonable medical judgement, determine that there appears to be virtually no hope for medical success, the staff will cease further CPR procedures. I understand that despite the best efforts of the doctors and staff at this facility, even the most successful CPR that may restore my pet’s life may not allow for my pet to regain normal mental and physical health.DNR. Please DO NOT RESUSCITATE my pet. DNR is a decision that CPR is not to be performed in the event that the pet stops breathing or has no heartbeat. If you chose DNR and your pet stops breathing or his/her heart stops beating then we will not attempt to revive your pet and your pet will pass away.
Release of Liability*

Regardless if whether I consent or decline to have CPR performed on my pet, in consideration for following my directive, I hereby waive, release and discharge any and all claims for damages, including, but not limited to claims for death, injury or property damage, whether or not resulting from the negligence, gross negligence, misconduct or other acts of Aspen Grove Veterinary Care, its veterinarians and staff, that I may have individually or on behalf of my pet, or that may subsequently accrue, as a result of honoring this directive, and I declare that any such veterinarian, staff and Aspen Grove Veterinary Care is acting in accordance with my directions. This is intended to be an advance release of legal liability, even if negligence or other misconduct occurs.
Photo Release

We will often take photos or videos of pets while in boarding or day care for the purposes of sharing with owners, to post on social media, the hospital website or include in printed publications. These photos will be shared daily to SmugMug so you can see what how your pet is doing during their stay. Failure to exercise the option to opt out, releases and discharges Aspen Grove Veterinary Care from any and all claims arising from the use of any photo or video.
