Commercial EV Charging Station Rebate Payment Request

In order to be eligible to apply for a rebate, you must have requested rebate pre-approval and received an award letter from CMLP prior to installation.

Before beginning your rebate payment request, review the section of CMLP's Commercial EV Charging Station Rebate Program Guidelines entitled "How to Request Rebate Payment," to ensure that you have assembled the information and documents required to complete this request form.

Please Note: The project electrician must obtain an electrical permit online prior to installation, and schedule an inspection by the Town's electrical inspector after the work is complete. More information about Town of Concord Building Department permits and inspections can be found in the Quality Assurance Section of CMLP's Heat Pump Rebates for Your Home page.  

Note:

• This rebate payment request must be received by CMLP within 90 days of the date on which CMLP verified that the station is operational.
• Depending on actual poject costs and the estimated or actual MassEVIP grant amounts received by the applicant, the CMLP rebate amount may differ from the CMLP pre-approved rebate amount.
