Welcome back! This extra copy request form is intended for returning clients only.

If you have not previously applied for and received a One Earth evaluation report and this is your first request, please return to the application page to fill out a new (initial) evaluation request.

Please proceed with this form if you have previously applied and received a completed evaluation report within the past 5 years and now need an additional report. Note that One Earth only keeps evaluation reports and credentials on file for 5 years. If your initial request was more than 5 years ago, you will need to begin a new (initial) evaluation request.

After all required information is entered, you will submit the completed form online and pay securely with a credit or debit card.
