Welcome back! This updated evaluation request form is intended for returning clients only.

Please proceed with this form if you:

Previously applied for evaluation based on incomplete/in-progress credentials and now have updated/completed credentials

This option is only valid for 1 year (12 months) from the initial evaluation request.

If you have not previously applied for a One Earth evaluation report and this is your first request, please return to the application page to fill out a new (initial) evaluation request.

After all required information is entered, you will submit the completed form online and pay securely with a credit or debit card.

If your name is different than it appears on your academic credentials (i.e. your name has changed due to marriage), please explain in the box provided below and e-mail us a copy of your proof of name change (i.e. marriage certificate).

Processing Time

Standard processing time is 5-7 business days. Time starts from receipt of all necessary credentials and does not include weekends/holidays. Rush service is available for additional cost.

Mail Service

Please choose how you would like any original credentials you submit returned to you. Please note that regular mail is a stamped letter envelope and is NOT tracked; once posted, it is in the hands of your local postal carrier and One Earth has no control over delivery. We strongly suggest choosing certified mail or courier for return of any original credentials. Please take a moment to make sure your postal address is complete and correct.

Additional Copy

If you require a copy of your evaluation report sent to more than one institution, employer or licensing board, additional copies can be requested for $20 each. Please indicate how many additional copies you would like and where you would like those additional copies sent.
