Liability and Personal Release
WARNING: As with any physical fitness program, dancing and dance exercises may result in injury. All students are urged to consult with their doctors regarding the advisability of dance lessons. Lois Ellyn Ballet Studio/Nouveau Chamber Ballet and its staff members assume no liability resulting from the student's/dancer's participation in our program and/or use of the premises, including but not limited to physical injury or illness. The student/dancer and/or his or her parent/guardian assume all risk of any injury while on the premises or traveling to or from the premises, including but not limited to physical injury or illness.
Please note that our studio is home to three resident cats. I further understand that the cats will roam freely in the class space. I understand and accept the risk of injury from interacting with the cats, including, but not limited to, scratches, bites, and allergic reactions. I understand and accept that cats may cause distraction during class/rehearsals and I do not hold Lois Ellyn Ballet Studio/Nouveau Chamber Ballet and its staff members liable for any injury sustained as a result of interacting with the cats.
I hereby give my permission to have me/my student’s likeness portrayed in photos, video, or film for the promotional or artistic use of Lois Ellyn Ballet Studio/Nouveau Chamber Ballet and its staff members. I hereby agree to read and comply with the rules and regulations of Lois Ellyn Ballet Studio. Lois Ellyn Ballet Studio/Nouveau Chamber Ballet is not responsible for personal belongings-please do not leave your items unattended.
