  • This form is to be completed by all athletes and officials who wish to be considered for a position on the Gymnastics SA State Representative Team, travelling to the Australian Gymnastics Championships being held May 5-24, 2023 on the Gold Coast.

    Athletes and ofiicials wishing to be considered for a position on the Gymnastics SA State Representative Team travelling to Border Challenge, being held June 24-29, 2023 in Brisbane need to complete this form.

    Completion of this form does not constitute selection on the team, please refer to the relevant gymsport selection policy/criteria for further information regarding the selection process.

    Please ensure that the information in the form matches that contained in the Gymnastics Australia online membership database. In any instance where this information does not match, you may be required to re-submit your nomination. Please ensure you or your club have/has entered your personal details correctly before completing this form.

    It is the responsibility of the person submitting the form (or parent/guardian if under 18) to check and ensure ALL personal details are entered correctly before submitting.

  • POLICIES AND DOCUMENTS

  • WORKING WITH CHILDREN CHECK

  • All Athletes over the age of 18 must have a current Working with Children Check (WWCC).

    All officials over the age of 14 must obtain a working with Children’s check (WWCC) **

  • COVID-19 VACCINATION POSITION

  • Gymnastics Australia have advised for all "Gymnastics Australia events, camps, national and international tours, meetings, courses and workshops, it is mandatory that staff, volunteers, participants and attendees are fully vaccinated in compliance with the relevant state and territory regulations and the specific requirements of the venues where such events, camps, national and international tours, meetings, courses and workshops". This is current as of February 4, 2022.

    Gymnastics Australia: COVID-19 Vaccination Position Statement 

    Please note all Gymnastics SA State Team members must meet these requirements in order to be selected. Gymnastics Australia will be providing further clarity around what "fully vaccinated" refers to and at what age ranges this mandatory vaccination position applies to. We will pass that to our members once we have that further information. Please note that proof of vaccination will be required in order for any State Team member to be complete the State Team Acceptance form, failure to provide this will deem the individual ineligible to be named as a member of the Gymnastics SA State Representative Team, unless a valid medical exemption is provided.

  • Completed online course certificates must be presented (email copy is ok) to the Team Manager or a GSA staff member.

  • (1) If you have completed The ASADA Level 1 Online Anti-Doping Course previously, you are not required do it again (the course does not have an expiry date)

    (2) Play by the Rules courses are valid for 2 years. If you have completed the relevant course within the last 2 years you will not be required to do it again.

  • GYMNASTICS SA REPRESENTATIVE TEAM AGREEMENT

  • I agree that if selected as a member of the Gymnastics SA State Representative Team travelling to the event on this nomination form, to travel, stay with, attend, compete and/or participate and perform my duties, fulfil my purpose and conduct myself at all times at the said event, and its associated trials or displays, meetings and official commitments as applicable.

    I accept the following responsibilities:

    • To remain under the authority of the Team Manager or person appointed by them from the date of assembly until my return to South Australia, or official release from the team.
    • To be prepared to participate in required training sessions/camps, meet prescribed departure times, training schedules, uniform requirements and other details as determined by Gymnastics SA, Team Manager or person appointed by them.
    • Time schedules as they pertain to departure dates, times, flights, training times and other requirements for team members shall be absolutely met, and further, adherence to such schedules shall be the responsibility of each individual team member, after having received such information from the Team Manager or person appointed by them.
    • To conduct myself in a proper manner and to offer myself in a prepared physical and psychological state which will ensure I can exercise to the best of my endeavours the duties, responsibilities and purposes for which I have been selected.

    It must be clearly understood by all team members that to breach any of the rules as set forth above may be potential cause for dismissal from the team, regardless of the method used for selection of that team. Such dismissal may result in the team member being sent home prior to the conclusion of the event, or depending upon timing, be removed from the team prior to departure. Any team member dismissed from the team will incur all costs associated with doing so.
