JGA Disabled Golf Cart Request Form

Please complete the form below by the close of entries to request a disabled golf cart for a specific Junior Golf Alabama tournament. All submissions are confidential and solely used to evaluate the golf cart request. If approved, JGA will arrange the cart request with the host club. Requesting person must pay the host club the appropriate cart fee each day of use.
