SECRET SANTA QUESTIONNAIRE
Please complete this questionnaire if you would like to participate in our FIRST Annual Secret Santa! This form is about YOU, be clear of what you want, and don't want, this will help who ever is your Secret Santa! By signing up for the Secret Santa this means you are going to be given someone's wish list to buy for them becoming their Sceret Santa, as well as someone will be yours!!Let's start with if you are choosing to be in the Secret Santa this form must be filled out by NOVEMBER 4th, then you will be placed into a draw I do privately and a message will be sent to you on November 5th of who you have to shop for. Once you receive their list you will have a week to return it to me with the payment which falls on November 11th and once everyone's orders are in I will submit the total order on November 12th to come to my home to put together every one's gifts. I'll be sending them all out to your Secret Santa in time to be opened at our December Gift Reveal on December 15th and also to keep everyone's addresses private. I will include a card & extra goodies on your behalf. Gifts should be no less than $35 and no more than $50 (before s/t). However if you CHOOSE to order more this is up to you. Shipping cost is $15 from my place to yours, please note I'm not charging anyone for the shipping cost from Tupperware to my home, you are only being charged once for shipping.
