Nos esforzamos por ser asequibles y accesibles, mientras ayudamos a los niños pequeños a crear la base para desarrollar fortaleza personal, confianza, integridad, inteligencia y creatividad en una comunidad solidaria.

Vision

Liability Release Form
Informed Consent and Acknowledgement
I hereby give my approval for my child’s participation in any and all activities prepared by Center for Creative Education. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Center for Creative Education and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected programs or events.In case of injury to said child, I hereby waive all claims against Center for Creative Education, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.

Media Consent and Acknowledgement
I hereby agree and give permission for Center for Creative education and/or partners to record, film, photograph, audiotape or videotape my/my child’s name, image, student work, and performance (hereinafter collectively referred to as “Works”) and to display, publish or distribute these Works for the purpose of publishing, posting on the CCE website, posting in CCE, posting on social media sites and/or for broadcasting on television or radio as determined by the CCE. I hereby waive any right to approve the use of these Works now or in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of these Works.
I understand that the Works may appear in electronic form on the internet or in other publications outside of CCE’s control. I agree that I will not hold CCE responsible for any harm that may arise from such unauthorized reproduction.
I also understand that external media organizations may attend events. I give permission for my/my child’s name, image, student work, and performance to be photographed, filmed, audio-taped or videotaped for the purpose of being published and/or broadcast on-line, on television or radio. *
