LEVEL 1-2 Classes. Students in this class are committing to 3-4 super fun and friendly competitions towards the later end of the year. If students enrol in the later part of the year they may join our end of year presentation performance then join the competitions the following year. Students perform individually, routines are at the basic fundamental stages of Rhythmic Gymnastics and have only the beginning skills of gymnastics alongside Rhythmic handling. Is the child on any medication which we should be aware? Please read below *
Ventolin and Epi-Pen medication must be available at all times in all occasions. Please describe below what medication this is and dosage. Emergency medications must be kept on site with staff. We have a staff locked medical storage cupboard where we must be able to obtain emergency medicine for your child at all times. A photograph of your daughter will be placed with this medication for emergency identification. If we are off site (eg competitions) it is the responsibility of the parent/gaurdian that all medication will be with the child. Do you suffer from any allergies that may lead to extreme discomfort but does not require an Epi-Pen ? This is also very important please explain in detail. If antihistamines are used please describe the dosage and medication , this will be kept in the locked medical storage cupboard.

Avanti Rhythmics runs public social media pages for consumer information and Advertising of our Rhythmic Gymnastics Club.

Other pages that share our information /photos and also use their own photos taken at competitions are Gymnastics NSW , Gymnastics Australia and photography company’s hired by Gymnastics NSW – Gymnastics Australia and Avanti Rhythmics for the purpose of selling professional photos to you as a member to the club or for advertisement of the sport.

Avanti is affiliated with both Gymnastics NSW and Gymnastics Australia Corporations and trusts their use, judgement and choice of social media products to be appropriate and meet the guidelines of child safety.

It is Avanti Rhythmics guarantee that only images that are appropriate to this sport and children will be used by Avanti and only for the purpose of Avanti Rhythmics and not for personal use.

No images be sent to other members or requests from anyone other than the parent of the child.

If you wish to check photos which may have other gymnast in them, you must come into the gym and we will show you for your consent. We cannot text you the photos for your consideration.

Consent

I give consent to Avanti Rhythmics Club to make, use and/or retain an image/s of the above-named Gymnast that may be displayed in their advertisement. I understand these images may be taken at a Avanti Rhythmics Hired training venue, competitions and /or any other location where the above named Gymnast is representing Avanti Rhythmics or are there for Avanti Rhythmics Club social gathering run by Avanti Rhythmics Staff.

Undertakings

I understand Avanti Rhythmics
- Will not use private addresses or information that may lend itself into unnecessary identification of an individual gymnast; a public location will be used.
- will not pay me for giving consent to use the above-named gymnast’s image/s;
- may keep the image/s on record unless I revoke my consent;
- will destroy images if I withdraw this consent, with exception of those already published;
- will not use images that show the gymnast in an uncompromising or suggestive position;
Privacy Statement: The personal information submitted on this form is collected by Avanti Rhythmics Club for obtaining your consent to use, disclose and publish your child’s image.

The information will not be disclosed to other parties except where permitted under the the Privacy Act 1988.

Please note social pages are continually monitored by Avanti Rhythmics and are handled with the up most of care.

- I understand the Term Fees are to be paid including annual Registartion in full before beginning lessons with Avanti
- I understand the late fee will be $30
- I understand the importance of dropping off and picking up my child on time and will be charged as stated in the fees policy if late
- I give my permission for my child to receive First Aid - Medical/Ambulance assistance in case of emergency and agree to pay such medical costs incurred.
- All information provided on this form is complete and correct to the best of my knowledge and I understand to advise the Club promptly of any changes that may occur.
- I will supply Avanti Rhythmics with any Asthmatic / Allergy/ Anaphylactic medications and agree they will be stored in a marked area in our first aid section with the childs photo attached all being supplied by you in a current and in date state
- I have read and understand the Fees Policy for Avanti in its entirety
- I Understand the classes are closed classes except for the 1st trial lesson
- Therefore, I understand the drop/off pick up policy
- I understand no food is to be shared or given to other children by you or other members between anyone.
- I understand the Avanti Rhythmics and Gymnastics NSW Code of Behavior for Gymnasts and Parents and Guardian’s
- I understand Avanti Rhythmics Bullying Policy and that we have a ZERO Bullying tolerance
- I understand and have read the Weather Emergency Policy
- You may not enter the hall at any point unless a staff member is present and greets you at the door
- I am aware that Avanti Rhythmics is a registered Covid Safe Business and has a Covid Plan in place that I have made myself familiar with and agree to participate in for the better health of all families.
- Example - I will not bring my child or self/siblings inside the centre if i/we are sick or have Covid
- Participation in Gymnastic activities carries with it a reasonable assumption of risk, i acknowlede and understand this.
In accordance with the Privacy Amendment (Private Sector) Act (2000), the information contained within this form will be used primarily for matters specifically related to participating in gymnastics and/or if a secondary purpose is related to the primary purpose and one could reasonably expect such use or disclosure.

If you choose not to provide personal information, we may not be able to provide you with the services you require including but not limited to emergency care that is directed and the level of service on which we pride ourselves.
