Details:Youth Community Basketball League

PLEASE READ CAREFULLY: Thank you for your interest in playing for the Pros Better My Game organization. Please realize this is a student athletic program and the focus on competition increases dramatically. The philosophy of this program is to give youth in America an opportunity to play sports at an advanced level while preparing them to play at the High School level.

By Signing Up Below I/We understand: There is a significant time commitment to your team and the program.

This is a recreational program and there is NO minimum playing time requirement. Most practices and games are important.  Missing practices and games will affect playing time. Sports comes after school functions (ie. band, orchestra, science fair, etc.), major religious functions, and major family functions (vacations, family reunions, etc.). While these practices are not mandatory, every effort should be made to attend in order to avoid falling behind the other players.

I/We the parent(s) of a player/student for a participation a Pros Better My Game, hereby give my/our child approval to participate in any/all activities during the upcoming season.

I/We assume all risks and hazards incidental to such participation including transportation to and from activities.

I/We do herby waive, absolve, indemnify and agree to hold harmless the Pros Better My Game organization, participants and persons transporting my/our child – except to the extent and in the amount covered by liability insurance.

 I/We understand Medical insurance will NOT be provided by the Pros Better My Game.

 I/We will furnish a certified birth certificate upon request.

PHOTOGRAPHIC RELEASE: I give Pros Better My Game Organization permission with respect to photographs, videos, motion pictures, and/or sound recordings being taken of my child to use, publish, and republish in the same, in whole or part, separately or in conjunction with other photographs or recordings.
I release and discharge Pros Better My Game Organization from any claims and demands arising out of or in connection with the use of such photographs, videos, motion pictures and/or recordings.

PLAYER’S CODE OF ETHICS:
• I will place my studies above athletics.
• I hereby pledge to show positive support for my teammates.
• I will show good sportsmanship to all players, coaches and officials at every game and practice.
• I will play to have fun but strive for success.
• I will remember that this is only a game. I will treat other players, coaches, visitors, and officials with respect.
• I will not use any foul or vulgar language at any time.
• If I have a problem, I will report it to my Coach, Director, board member and/or parents before it becomes a bigger problem.
• I will conduct myself in a respectable manner when at tournaments or traveling to other facilities.
• I understand that being part of the Pros Better My Game Sports Travel Team is a privilege that can be taken away for not abiding by this code of ethics.
• I accept responsibility for my behavior on an off the court. I understand that what I do and say affects my teammates, my school, and other people either positively or negatively.
• I will always exhibit Pros Better My Game values of Responsibility, Respect, Honesty, and Positivity.

PARENT’S CODE OF CONDUCT:
• I hereby pledge to provide positive support, care and encouragement for my child participating in the PROS Basketball Travel Team by following this Parent’s Code of Ethics.
• I will encourage good Sportsmanship by demonstrating positive support for all players, coaches and officials at every game, practice or other event.
• I will place the emotional and physical wellbeing of my child ahead of a personal desire to win.
• I will support coaches and officials who are here to provide a positive and safe environment for my child.
• I will serve as a role model for our players, talking politely and acting courteously toward coaches, officials, other parents, visiting team parents, and spectators at practices, games and meetings.
• I will keep competition and winning in perspective. I will understand and support positive encouragement for my child and all players.
• I will help my child to properly prepare for practice and games, by having the appropriate equipment and arriving to practices and games on time.
• I will help my child to focus on developing as a player and a person.
• I will always exhibit the Pros Better My Game values of Responsibility, Respect, Honesty and Positivity.

All fees are due at the yearly kickoff meeting or monthly (as agreed upon by Pros Better My Game.)

By signing below, I agree to the following.
I will not hold Pros BMG responsible for any accidents or injuries that may happen to my child during transportation, practice, or games. I agree that Pros BMG holds the rights to all photos taken and may use them for marketing purposes. I commit to having my child ready and prepared for all games and practices and will do my best to transport my child to those games and practices. I commit to being a positive team parent, saying positive things to my student, teammates, coaches and referees. I will not speak poorly of or disrespectfully to coaches and referees.
