1. Initiate the background procedure required by MLC's Youth and Education Standards(YAS) for any adult leader of a ministry activity involving minors if one is not already on file in our Church Member database.

2. It will inform Youth & Education Leadership of your willingness to serve, where you want to serve, and how much time you have to serve.

3. It will allow you to optionally share gifts or special skills God has given you that you are willing to share to enhance MLC's ministry to children and youth.

Concerning the Background Check: After you submit this form, if you do not already have a background check on record at MLC, MLC Office Staff will initiate a background check. You will receive an email from Trusted Employees.Com with a link to complete the screening online. Only you and MLC's pastor will see the results. Once your background check is completed it will be noted in your MLC record

As a volunteer in service to the congregation I agree to:
- Abide by all church policies regarding the ministries of this church
- Follow all policies and procedures of MLC's Youth Activity Standards
- Report promptly any observed abusive or inappropriate behavior to the appropriate church and/or government authority.
