100 Heroes Nomination form
As part of our centennial celebration, United Way of Trumbull County invites you to nominate someone who has inspired you by making a difference in your life, the lives of others, or the community at large. If possible, please submit a photo of a hero but a photo is not required at the time of submission. You may remain anonymous if you choose, but we may ask you for additional information regarding your nomination. Visit unitedwaytrumbull.org and our Facebook page for updates. Nominees will be recognized beginning in March and throughout the year. Thank you for participating!
