Tattoo Appointment Request

Hello , and thank you for your interest in scheduling with me ! Here are a few key things to note that will help the both of us get you scheduled easily ! After reading through this information , make sure to answer every question and fill out every section in this form to the best of your ability .

▪️I price my pieces based on size , style , complexity , and placement .

▪️any type of session work (sleeve work , large scale pieces like backs and full chests , or any type of black work or blackout) can be scheduled by either :

•a half day session (4 hours for $800)

•a full day session (8 hours for $1400) (full days start earlier than normal hours, and would have a start time of 10am)

▪️shop minimum is $100

▪️I REQUIRE a MINIMUM of a $100 NONREFUNDABLE retainer to schedule any and all appointments . More info on my retainer policy is at the bottom of this form and requires a signature that you understand and accept it .

***RETAINER AGREEMENT*** To schedule an appointment with me (Maura.ink) a minimum of a $100 NON REFUNDABLE retainer is required. The retainer will come off of the overall price of the tattoo, the day of your appointment, upon completion. If you would like to put a higher amount down as a retainer, you may, but whatever amount you put down will be NON REFUNDABLE, and still come off of the price of the tattoo, the day of, upon completion. You can reschedule and not be required to pay another retainer if I (Maura.ink) am notified BEFORE 48 hours (2 days) of your original appointment date, and still get the retainer taken off the overall price of the tattoo, the day of the new appointment date, upon completion. Any appointments rescheduled within 48 hours (less than 2 days) of your appointment date, will require another minimum of a $100 NON REFUNDABLE retainer, leaving the first retainer void. By signing this you have read, understand, and agree to the retainer agreement.
