    Please fill out this form in entirety to ensure we can provide your pet with profound personalized care. If you need to cancel/reschedule your pet's procedure, please provide a 48 hour notice to allow us to provide care to another patient in need. A cancellation fee of $100/hour reserved will be billed for surgery appointments without a 48 hour cancellation/reschedule notice.
  • It is imperitive that we be able to reach you in a timely manner while your pet is admitted for surgery today.

  • For the patient's safety, our doctors recommend spaying your pet at least 8 weeks after the last day of their heat cycle.

    If the patient's last day of their heat was not 8 weeks ago, there may be additional costs for their procedure or we may have you reschedule. There is increased risk for intra-operative and post-operative hemorrhage for pets spayed in close proximity to their heat cycles.

  • Dental Extraction Consent Form

    I understand that some of the dental disease and pathology for my pet may not be revealed until the complete oral examination and radiographs have been completed.  If additional treatment is necessary, I understand that any possible extraction can range from $20 - $300/extraction.

  • CPR CONSENT

    In the event of an unforeseen emergency, Urban Veterinary Associates will attempt to reach you without delay. Please know that we will take every precaution to ensure that your pet is safe and healthy enough to undergo their procedure today. Any known risks will be discussed with you. However, very rarely, emergencies do happen and we want to know your preference if no one can be reached. 

    Should your pet require cardiopulmonary resuscitation (CPR), including cardiac compressions, positive pressure respiration, emergency medications, or other heroic interventions, would you request that the doctor at Urban Veterinary Associates conduct such medical care?

    If you request such emergency procedures, you agree to be held responsible for veterinary services provided to your pet while staff members pursue treatment and try to reach me for further directions. Regardless of your pet’s recovery or survival, you agree to pay CPR fees in addition to other fees already identified by the practice and agreed upon by me.
