RELEASE AND INDEMNITY AGREEMENT
I, {fullName}, do hereby freely and willfully and for good and valuable consideration, including but not limited to, the opportunity to bring guests with me to this Awards Dinner do hereby enter into this Agreement.
I do hereby release, remise, discharge, forgive and excuse any and all claims, demands, actions, causes of action, liability or obligation, foreseen or unforeseen, of or against Citizens Finance of Fort Payne, Inc., Citizens Finance of Guntersville, Inc., Citizens Finance of Huntsville, Inc., Community Credit of Leeds, Inc., Citizens Finance, Inc., Citizens Finance of Oxford, Inc., Community Credit, Inc., Citizens Finance of Sylacauga, Inc., Citizens Finance of Tuscaloosa, their affiliates and all of its and their shareholders, directors, officers, employees and agents, and their family members, heirs, executors, personal representatives, successors and assigns of all of the foregoing (collectively, the “CFG”), resulting from my or my invitees’ and guests’ activities related to the permission granted to bring guests with me to the Awards Dinner.
Further, I do hereby agree to indemnify and hold CFG harmless from any and all claims, actions, and/or causes of action, which may be suffered by me or my invitees or guests; and in the event of any such claim, action or cause of action is asserted against CFG by any of the foregoing, I will pay all costs related thereto, including but not limited to, claims for damages or injury, and the cost of defense of said claims. The Release and Indemnity extends to everyone encompassed in the term “CFG.”
The permission granted by CFG is personal to me and is revocable by CFG at any time.
