Ear Piercing Registration Form

Must be ages 11-65+. Parental consent required for anyone under 18-years-old. Appointments are required. 

We offer ear piercing services for a flat fee of $49.99, which includes a pair of earrings, cleansing gel, and care instructions.

Our friendly piercing specialists wear single-use disposable gloves and use tools that are cleaned before and after every use. We use pre-packaged and pre-sterilized earrings, and clean your ears with disinfectant prior to piercing. To minimize discomfort, we pierce both ears simultaneously and use non-allergenic, nickel-free earrings.

Parent or gaurdian must be present for anyone under the age of 18.

I acknowledge the following statements:

• I understand my ears will be pierced with single-use earrings that are pre-packaged, sealed, and sterilized.
• If I am taking blood thinning medications or antibiotics, if I have diabetes, if I am pregnant, or have a history of infection or any other medical condition, I acknowledge that ear piercing may carry a risk and must consult a physician for approval before piercing my ears.
• Despite the best efforts of Ella Community Pharmacy, STUDEX, and my own aftercare efforts, I understand the potential for infection exists. Improper aftercare, poor hygiene, metal sensitivity, or other causes may increase the risk of infection.
• I understand that ear piercing may contribute to the formation of cysts or keloids.
• I understand that STUDEX and Ella Community Pharmacy is unable to monitor my at-home aftercare. Therefore, it is my sole responsibility to follow the Ear Piercing Aftercare Instructions provided at the time of piercing.
• I agree to communication from Ella Community Pharmacy via phone and email.
