I acknowledge there is a fee associated for this service. The initial fee is $175. The fee may be higher depending on the complexities and or time needed to fufill the requst. I understand I am responsible for this fee, and request the fee be added to my account. I uderstand the fee must be paid upon receipt and may incur late fees if the balanace is not paid in full.
