• Tillamook K9 Rescue Adoption Application

  • Tillamook K9 Rescue is a volunteer run, non-profit, animal welfare organization dedicated to rescuing, reuniting found dogs with their owners, or placing these dogs in their new forever home. 

    Tillamook K9 Rescue is funded in part through fundraising efforts and public donations. Found dogs are cared for at our small shelter facility or by volunteer foster homes and receive complete medical care, which can include worming, vaccinations, spaying/neutering and microchipping prior to their adoption.

  • This questionnaire is designed to help Tillamook K9 Rescue and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. Tillamook K9 Rescue reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!
