A) REGISTRATION FIRST ENROLLMENT ANNUAL FEE FOR THE FIRST-YEAR PROGRAM FOR NEW PLAYERS ENROLLMENT.

B) COSTS OF INSTALLMENT PAYMENTS PER AGE GROUP Payment must be made without any interruption MONTH BY MONTH to avoid training suspension or registration cancellation for non-payment.

E) SELECT TRAINING UNIFORM SIZE: Costs $75.00 upon registration. Uniform: 1 Set with 1 Jersey, 1 shorts, and 1 pair of socks is provided once, upon first admission. Uniforms are mandatory to enter the playing field as part of athletes' discipline. No Uniform - No Training. Parents are responsible for bringing players wearing the complete set without any alterations or damage.

C) SUMMER SEASON OPTION. THIS OPTION IS ONLY FOR SEASONAL PLAYERS! ONLY 10 TRAINING SESSIONS AT A TIME COMPLETED WITHIN A MONTH of 30 days consecutives. OTHER PLAYER'S CHOOSE FIRST OPTION: SKIP

D) If you choose seasonal or single training session please add here your 8 first training dates (or one date for a single training) schedule starting on Mondays, Wednesdays, and Thursdays within the period of 30 days.

Important Notice: Your Youth Training Academy and Development Program registration will expire one year from the month of admission. The annual renewal will take effect 30 days prior the month players will complete the year program.
