Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Via Papera LLC DBA Arte al Sole during the selected activity. In exchange for the acceptance of said child’s enrollment by Via Papera LLC DBA Arte al Sole, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Via Papera LLC DBA Arte al Sole. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Via Papera LLC DBA Arte al Sole, including all coaches and affiliates, teachers, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.

I hereby voluntarily, at my own risk, sign this Informed Consent and Acknowledgment and acknowledge and understand that there may be risks associated with the activities, which have been fully explained to me. I hereby agree to abide by all rules, instructions, policies and procedures imposed by the Releasee. I further agree to indemnify and hold harmless the Releasee, its employees, agents, officers, from and against any and all liability incurred as a result of or in any manner related to my participation in the activities.

I hereby certify that I am of legal age and competent to execute this Informed Consent and Acknowledgment, that in doing so of my own free will and accord, voluntarily and without duress, and that I do so intending to bind myself, my executor, my heirs, and administrators or assigns to the fullest extent.

I have read and understood the foregoing, and acknowledge my consent to the terms of this Informed Consent and Acknowledgment by signing this Consent.
