    MUSIC LEVEL 3: LARGE CONCERTS, STAGE EVENTS, ETC. (OFFICIAL SOUND TECH REQUIRED - NOT SUPPLIED BY THE BUNKER) - $200 (NOT USING AN OFFICIAL SOUND TECH WILL RESULT IN LOSS OF DAMAGE DEPOSIT) CANTEEN USE (COOLER, COFFEE MAKER, POP-CORN MACHINE - YOU BRING YOUR OWN DISHES, CUTLERY, & COFFEE SUPPLIES) INCLUDED IN BASE FACILITY COST

    FULL KITCHEN RENTAL: USE OF COMMERCIAL KITCHEN (CONVECTION OVEN, STOVE TOP, COMMERCIAL DISHWASHER, DISHES & CUTLERY (FOR 100 PEOPLE), ETC

    SET-UP/CLEAN-UP DAY(S): $100 PER DAY PRIOR/POST YOUR EVENT FOR SET-UP/CLEAN-UP PURPOSES

  • NAME OF SECONDARY:

    IF SIGNED APPLICANT CANNOT BE PRESENT DURING THE TIME OF THE RENTAL, A SECONDARY APPLICANT MUST BE ON SITE.
    • Rentals are confirmed only AFTER rental form & damage deposit of $100 (not to be applied to facility cost) have been received by a Bunker staff member.
    • Full payment is required no later than 2 weeks prior to the rental date.
    • Cancellation of rental with less than 7 days notice will result in the loss of your damage deposit.
    • Failure to arrive 1 hour after scheduled arrival time will result in the loss of your damage deposit.
    • The Bunker reserves the right to cancel your event no later than 7 days prior to your event without prior notification (You will receive a full refund)
    • Any damages that happen over the time of your rental will come out of the damage deposit.
    • Damages exceeding the $100, are to be paid in full. Should no damage or loss occur, your damage deposit will be returned to you.
  • WAIVER & RELEASE FROM LIABILITY

  • I,         (PLEASE PRINT YOUR NAME) ON BEHALF OF MYSELF, MY EXECUTORS, ADMINS, HEIRS, NEXT OF KIN, SUCCESSORS AND ASSIGNS; HEREBY WAIVE, RELEASE, AND DISCHARGE FROM ANY AND ALL CLAIMS OR LIABILITY FOR DEATH, PERSONAL INJURY, PROPERTY DAMAGE, THEFT, OR DAMAGES OF ANY KIND. WHETHER OR NOT ATTRIBUTABLE TO THE NEGLIGENCE OF THE RELEASES, THE BUNKER YOUTH MINISTRY STAFF, VOLUNTEERS, DIRECTORS, AND ANY AGENTS, SUCCESSORS, OR ASSIGNS OF ANY OF THE FOREGOING WHICH MAY ARISE OUT OF THE USE OF THE BUNKER FACILITIES OR EQUIPMENT.

    I HEREBY AGREE NOT TO SUE ANY OF THE PERSONS OR ENTITIES MENTIONED ABOVE FOR ANY OF THE CLAIMS OR LIABILITIES THAT I HAVE WAIVED, RELEASED, OR DISCHARGED HEREIN.

    I AGREE TO INDEMNIFY AND HOLD HARMLESS THE ORGANIZATION OR ANY PERSON OR ENTITY MENTIONED ABOVE FROM THE AGAINST ANY AND ALL DAMAGES, LOSSES, LIABILITIES, OBLIGATIONS, PENALTIES, CLAIMS, LITIGATION, DEMANDS, DEFENCES, PROCEEDINGS, COST, DISBURSEMENTS, OR EXPENSES OF ANY KIND OR NATURE WHATSOEVER (INCLUDING, WITHOUT LIMITATION, ATTORNEY'S FEES AND EXPERTS FEEL AND DISBURSEMENTS) WHICH MAY AT THE TIME BE IMPOSED UPON, INCURRED BY OR ASSERTED OR AWARDED AGAINST ANY OF THE RELEASES IDENTIFIED.

    I HEREBY AGREE TO ALL RENTER RESPONSIBILITIES AND GENERAL POLICIES LISTED ON PAGES 2 & 3.

    I HEREBY AFFIRM THAT I AM AT LEAST EIGHTEEN YEARS OF AGE OR OLDER, AND THAT I HAVE READ AND UNDERSTAND.

  • GENERAL POLICIES

    Failure to follow these rules may result in loss of damage deposit and/or ban on future rentals
  • 1. ABSOLUTELY NO ALCOHOL IS ALLOWED ON THE PROPERTY.
    2. POOL TABLES ARE NO CHAIRS OR COUNTERS
       - NO FOOD OR DRINK ON/AROUND THE POOL TABLES
       - CUES/BALLS ARE FOR PLAYING POOL ONLY
       - POOL TABLES ARE NOT MEANT TO STORE PERSONAL ITEMS (JACKETS, PURSES, ETC
    3.WE ASSUME NO RESPONSIBILITY FOR LOSS/DAMAGE TO PERSONAL OBJECTS AND/OR INJURY TO PERSONS DURING THE RENTAL (INCLUDING, BUT NOT LIMITED TO DISFIGUREMENT, DISMEMBERMENT, AND DEATH)
    4. SMOKING/VAPING IS ALLOWED ON THE PROPERTY, BUT ONLY IN THE DESIGNATED OUTDOOR SMOKING AREA (MINIMUM OF 5 METERS/16 FEET FROM ANY ENTRANCE OR EXIT) DESIGNATED AREAS INCLUDE: PATIO AREA THAT HAS 'BUTT OUT' STATION, PARKING LOT, AND FIT PIT AREA CIGARETTE/CIGAR BUTTS, PIPE ASH, VAPE COILS/COTTON ARE TO BE DISPOSED OF CORRECTLY (BUTTS IN THE BUTT-OUT STATION, PIPE ASH IN THE FIRE PIT, VAPE COILS/COTTON IN THE OUTDOOR TRASH CAN)
    5.THE SIGNED INDIVIDUAL WILL BE HELD 100% RESPONSIBLE FOR ANY ISSUES, LOSS, OR DAMAGES THAT ARISE.
    6. ABSOLUTELY NO ONE OR ANYTHING ON THE STAGE UNLESS OTHERWISE AGREED UPON. THIS INCLUDES THE RAMP. EVEN WITH PERMISSION, ABSOLUTELY NO FOOD/DRINKS ON THE STAGE AT ANY TIME DO NOT ALLOW CHILDREN TO RUN ON THE RAMP OR JUMP OFF OF THE STAGE
    7.ABSOLUTELY NO ONE IS PERMITTED IN THE UPSTAIRS OFFICE AREA OR AREAS THAT HAVE NOT BEEN AGREED UPON.
    8.THE BUNKER'S PERSONAL SUPPLIES (DRINKS, CANDY, FOOD, SPICES, ETC ARE ALL OFF LIMITS, UNLESS YOU HAVE REQUESTED TO USE THE SUPPLIES. SUPPLIES ARE TO BE PAID FOR AND ANY MONEY MADE FROM THE SALE OF THESE SUPPLIES WILL REMAIN POSSESSION OF THE FACILITY

  • RESPONSIBILITIES OF RENTER

  • 1. CLEAN ANY/ALL AREAS USED, UNLESS OTHERWISE INSTRUCTED. EACH ITEM MUST BE DONE REGARDLESS OF WHETHER YOU THINK IT NEEDS TO BE DONE OR NOT (LIST BELOW)

    • ALL TABLES & COUNTERS ARE WIPED WITH A WET CLOTH
    • FURNITURE IS PUT BACK EXACTLY WHERE YOU FOUND IT
      • 4 TABLES PER SPOT UNDER THE WALL
      • NO MORE THAN 30 CHAIRS LEFT OUT AND STACKED AGAINST EAST WALL NEAR FIRE EXIT
      • COUCHES AND CHAIRS ARE PUT BACK INTO THE CORRECT AREAS
    • FLOORS IN MAIN AREA & GYM ARE SWEPT
    • ALL CARPETS IN THE BACK ROOM ARE VACUUMED
    • ANY DISHES USED ARE WASHED AND PUT BACK IN THE APPROPRIATE SPOT
    • DISH CLOTHS & TOWELS ARE PUT IN THE BASKET IN THE COMMERCIAL KITCHEN
    • ALL GARBAGES (2 IN MAIN AREA, 2 IN GAMES AREA, 1 IN COMMERCIAL KITCHEN) HAVE BEEN TAKEN OUT (DUMPSTER TO THE SOUTH OF THE PARKING LOT) AND A NEW BAG IS PUT IN
    • RECYCLING HAS BEEN REMOVED FROM THE BUILDING AND BROUGHT TO THE BOX BY THE PARKING LOT ENTRANCE.
    • BATHROOMS ARE CHECKED & TIDIED IF NECESSARY
    • ALL STAGE/GYM EQUIPMENT HAS BEEN PROPERLY PUT BACK WHERE IT CAME FROM
    • BOARD/VIDEO GAMES, CONTROLLERS, POOL TABLES AND CUES, ETC. ARE CLEANED UP AND PUT AWAY IN THEIR PROPER SPOTS
    • PATIO AREA & GAZEBO is TIDIED UP FROM YOUR MESS
    • COFFEE MAKER is PROPERLY TURNED OFF (INSTRUCTIONS ON THE MACHINE)

    2. NOTIFY THE RENTAL HOST ONCE YOU ARE ABOUT TO LEAVE (ALYCIA'S PHONE NUMBER IS ON THE 1ST PAGE OF THE RENTAL BINDER. FEEL FREE TO TEXT)

    3.TURN OFF ALL OF THE LIGHTS (INCLUDING BACK ROOMS & STORAGE ROOMS - EMERGANCY LIGHTS IN GAMES AREA CANNOT BE TURNED OFF)

    4. TURN OFF ALL TV'S, PROJECTORS, ETC,

    5. REMOVE ALL GARBAGE/RECYCLING FROM THE BUILDING AND PUT IT IN THE PROPER DISPOSAL AREAS

    6. LOCK THE FACILITY WHEN YOU ARE DONE FOR THE NIGHT. FORGETTING TO DO SOWILL LEAVE YOU 100% LIABLE FOR AND LOSS, DAMAGE, OR ISSUES THAT ARISE DURING THE UNLOCKED PERIOD.

    7. IF YOU HAVE USED THE LOCK-BOX KEY TO GAIN ENTRY TO THE BUILDING, PLEASE PUT THE KEY BACK INTO THE LOCK BOX!

    8. IN CASE OF DAMAGE TO THE BUNKER'S PROPERTY, ITEMS, ETC. PLEASE NOTIFY THE BUNKER IMMEDIATELY! FOR DAMAGES EXCEEDING THE DAMAGE DEPOSIT, THE SIGNED RENTER WILL BE HELD ACCOUNTABLE AND ASKED TO COVER THE COST OF REPLACEMENT OR REPAIR. THE BUNKER WILL SOURCE REPLACEMENT/REPAIRS AND CONTACT YOU WITH THE AMOUNT OWED. THIS INCLUDES: ANY DAMAGE OR LOSS TO: VIDEO GAMING EQUIPMENT, POOL TABLES, CUES, ELECTRONIC DEVICES, BUILDING DAMAGE (WALL DENTS/SCRATCHES, FLOOR DAMAGE, CARPET TEARS, FENCES, RAILINGS, ETC, COMMERCIAL KITCHEN EQUIPMENT, ANY APPLIANCES, ETC. SECURITY CAMERA FOOTAGE WILL BE VIEWED TO PROVIDE PROOF OF DAMAGE UPON REQUEST

    9. ALL SURFACES ARE WASHED & WIPED DOWN

     
    COMMERCIAL KITCHEN ADD-ON

    10. ANY DISHES HAVE BEEN PROPERLY WASHED, SANITIZED, DRIED, AND PUT AWAY ACCORDING TO HEALTH DEPARTMENT STANDARDS (PROPER STANDARDS ARE POSTED IN THE KITCHEN)

    11. GARBAGE HAS BEEN REMOVED AND PUT IN THE PROPER DISPOSAL LOCATION

    12. ALL FANS & LIGHTS ARE TURNED OFF

    13. ALL APPLIANCES (STOVE TOP, OVEN, DISHWASHER, ETC ARE PROPERLY TURNED OFF
