This form is a guide to assist with Safety Regulations Part II, Cat 4 & 5 for voluntary self-declaration of compliance. Spot checks may be carried out by a sailing committee member or delegate at any time.

This form is valid for a maximum of one year, or less if inspection identifies that improvements are required.
