With this form I will be able to determine that your appointment is booked properly. If we are a good fit, I will contact you in 24-48hr to proceed with scheduling your appointment.   • SERVICE RE-DO POLICY: I strive to offer my guests the highest level of guest satisfaction. If you are having challenges with your cut or color , let me know within 7 days of your visit and I'm happy to correct the issue with no additional charge.   • FUTURE APPOINTMENT CANCELLATION POLICY: In order to continue providing the best possible scheduling options to my guests, I do require 24 hours notice should you need to cancel or reschedule your visit with me. Should you cancel your appointment with less that 24 hours notice, a $50 per service cancelled fee does apply and will be due before a new appointment is scheduled
