Required Reporting of Bank Account Information
This form must be completed by all GSMISTS Service Units/Troops that have been given permission to open a bank account in the Council's name. This form must be completed for new bank accounts as soon as it is
opened or if there are any changes in authorized signers.
The information below will be available to your Service Unit Treasurer and GSMISTS staff. Upon submission your form will be sent to customercare@gsmists.org.
