DEMOGRAPHIC INFORMATION

In order to better serve our community, be able to focus our efforts and be able to accurately apply for grants, we are trying to better understand who we are serving. Please fill this out as accurately as possible with regards to the team.

We will be using the demographic information collected here in aggregated form only with no personally identifiable information released.
