Student and Instructor ID Form

Please submit the following information including a headshot image for ID badge.

All students and instructors MUST obtain a ProMedica ID badge.

Options include:

• Your ID will be sent to the security office of the hospital you select on the ID request form for you to pick up

Important Information:

• Student cost is $10.00 which is non-refundable. Cash or Check (to ProMedica)
• Card will be valid during all approved clinical rotations until graduation.  If card is lost, there is a $10.00 replacement charge
• ID cards should be returned at the end of your clinical time.  You may mail the card back to Security in the interdepartmental mail on your final clinical day

THIS FORM MUST BE COMPLETED 72 HOURS PRIOR TO ARRIVING FOR YOUR ID.

PLEASE ENSURE YOU HAVE A PROMEDICA ID# PRIOR TO ARRIVING FOR YOUR ID.

(This ID# can be obtained from your school/coordinator)

BADGE PHOTO SUBMISSION

Please upload a photo for your badge. Image must be forward facing, head/face (shoulders and above), PLAIN, LIGHT COLORED BACKGROUND, (i.e. passport picture, driver's license) and NO FILTERS. See example and photo link below.

 

DO NOT take photos of badges, drivers licenses, or while sitting in your car.

These photos do not come out clear enough for our badges. Photos taken in your car do not provide a plain background. Inappropriate submissions will be denied and delay badge printing.
