  • Have you ever had eyelash extension before?*

  • Do you often rub, pull, or pick your lashes?*
  • Are you at least 18 years of age or older?*

  • ♡ PRIOR TO YOUR APPOINTMENT ♡

    • I do not take foreign fills from another lash artist.
    • Please make sure to come with a clean face/lashes. Meaning no eye makeup, no glue and no serums/lotions.
    • I recommend all my clients to shower prior to their appointment.
    • Be prepared to be here for 2.5-3 hours. Please DO NOT rush me. Lashes are a luxury and are tedious. 
       Feel free to bring comfy clothes, your own headphones and blanket!
    • Please come on time to your appointment.
    • Please no cell phone use meanwhile getting your lashes done.

    1. A $30 deposit is required in order to secure your appointment. Deposits also go towards your service.
    2. The deposit will be collected by the booking site. Once you have received the text and/or email, you're all set! 

     THE DAY OF YOUR APPOINTMENT

    1. The day of your appointment PLEASE BRING THE EXACT AMOUNT OF CASH FOR YOUR REMAINING BALANCE. If not, an extra $10 fee will be added on top. 

    2. Upon your arrival, please TEXT OR DM ME that you are outside.

  • ♡ CANCELLATION/RESCHEDULING ♡

    • If you must cancel or reschedule, please do so at least 24 HOURS before your appointment. Appointment cancellations may be done through GlossGenius, text or DM.
    • If you reschedule, your deposit will follow you for up to ONE MONTH. After a month, a new deposit will be required.
    • NO refunds. 
    • If you no call/no show after 20 minutes you will NO LONGER be able to book with me.

     

    ♡ LATENESS ♡

    • There is NO LATE FEE. However, your appointment time will be cut by the amount of "x minutes" that you are late.

  • ♡ POLICY AGREEMENT ♡ 

    ♡ PHOTO AND VIDEO CONSET ♡

    1. I understand that lashes may take up to 3 hours.
    2. I understand that it is my responsibility to keep my eyes closed the entire procedure until my lash artist addresses me to open my eyes.
    3. I understand that some risks of this procedure may be, but not limited to eye redness and irritation due to the fumes from the adhesive.
    4. I agree that by reading and signing this consent form, I release Adrianna Marroquin (L.A. Wink Lashes) from any claims or damages of any nature.
    5. I am sound mind and capable of executing this waiver for myself.
    6. I give Adrianna Marroquin permission to show my before and after photos of eyelashes to other potential clients. 
    7. I have read and completed the consent form in it's entirety, and have answered everything to the best of my ability. I have been informed of potentially harmful or negative side effects that could be caused by the application and/or removal process. 
    8. Cancellation policy is fully understood.
